Process: Add a Feedback Form to a Party Record

In Process: Add/Edit a Feedback Form, we discussed how a user can add a Feedback Form into their system. In this article, we will cover how to add these Feedback Forms against a Party Record via the use of New Forms and Shared Forms.

 

Adding a New Feedback Form to a Party Record

To Add an New Feedback Form against a Party Record, follow these steps:

  • Navigate to the Party Record that you want to add the feedback form against.

  • From the left-hand menu of the Party Record, navigate to Feedback Forms → List & New.

  • Click the + New button and then select Add New Form.

  • This will take you to the New Form configuration page.

    1. Form - This allows you to select from the list of all Feedback Forms that have been created in your database, regardless of whether they have been made available to all individuals.
    2. Category - This is the Feedback Form Category. This field will by default, be populated with whatever Category was set up during the Feedback Form creation, but this can be overwritten if required.
    3. Assignee - Choose whether to assign a system user to the Feedback Form. When the individual responds to the form, this is the user who will be assigned.
    4. Available From - Here you can choose from what date you want this form to be available for use by the individual. If the individual attempts to access the form URL before this date, they will be presented with an error message.
    5. Available To - Works in much the same way as Available From, but conversely allows you to choose what date the form is available to. If the individual attempts to access the form URL after this date, they will be presented with an error message.
    6. Allow Saving - If Allow Saving is enabled, a student can save the Form and submit it later. When disabled, the student must complete the feedback form in one session by clicking the Submit button.
    7. Save Expires After (h) - If Allow Saving is enabled, this is an additional setting that allows you to dictate how long after the student has saved the Form, that the Form will expire. The default value is blank, meaning that the Form will never expire.
    8. URL - This is the URL that will be used, by the system, to take the students to a unique version of the Feedback Form. You can edit the end of the URL to be a string or number value of your choosing, but you cannot change the remainder of the URL.
    9. Enabled - To use this form, ensure the Enabled box is checked. Otherwise, this Form will not be available for use by the individual, regardless of whether the URL has been generated.

  • Click on the Save button once you have completed these settings. The form and its URL will now be listed against the Party Record.

You have now added a New Feedback Form against a Party Record.

Adding a Shared Feedback Form to a Party Record

To Add an existing Shared Feedback Form against a Party Record, follow these steps:

  • Navigate to the Party Record that you want to add the feedback form against.

  • From the left-hand menu of the Party Record, navigate to Feedback Forms → List & New.

  • Click the + New button and then select Add Shared Form.

  • You should find that you are then presented with all Feedback Forms in your system that are configured as available for all individuals, as well as a Tick Box icon and an Edit icon for each form.

  • The Tick Box icon represents the Add form button, which will auto-generate a unique URL for the individual and add the form using the settings that have previously been configured during Feedback Form creation. Click this button if you are happy to use the existing Feedback Form configuration.

  • If you are wanting to customise the Feedback Form settings before adding it to the Party Record, you can click on the Edit icon which represents that Customise and Add Form button. This will bring up the New Form window, previously discussed above. By default, these settings will be pre-populated with the settings that were configured during Feedback Form creation.

  • The form should now be listed as available for the Party Record, with the URL provided. This will be a unique URL for each individual that you are wanting to add the form against, meaning that the information that they provide will be specific to their Party Record.

You have now added a Shared Feedback Form against a Party Record.
